Instructions on how to fill in the Examination and Estimation Request Form

• Fax Transmission Date (Required Item)
• Inquiry Content (Required Item)
• Requested Reply Date

  Please fill in the requested reply date with respect to your submitted inquiries.

• Contact Information

  Required Items: Company, Name, Telephone

• Product Number and Product Name

  Please specify if there is a certain product number and/or product name you would like to request.

• Purpose

  Please fill in as much detail as possible. (Example: Attachment Process or Post Process)

• Standard Quantity

  Please fill in either the monthly production output (per month or per lot) or annual output quantity.

• Classification, Material Type, Density, Shape Opening (Required Items)
• Shape

  Please either fill in the rough sketch diagram provided or attach your own diagram to the form.

Instructions for saving your request form

• Please click on the Examination and Estimation Request Form (Fax Form).
• A new window will open.
• Click on the Save As button from the toolbar (Floppy Disk icon).
• You will see a dialog box for saving your form.
• Please enter the save location and file name of the form.
• Click on the Save button.
• E The Ready-Made Spring Case Order Form is now saved on your computer.

Instructions on how to print out your saved request form

• Use either My Computer or Explorer to find and display your saved order form.
• Double-click the saved order form.
• Acrobat Reader will now start.
• Click on the Print button (printer icon) from the toolbar.
• A print dialog box will be displayed.
• Click on the OK button.
• The order form will print on your default printer.
